Intake evaluation – 90 minutes, $295
Conducted at the first session, preferably with parents only

Psychoeducational evaluation - $3000
When you’re trying to determine the root cause of your child’s difficulties and want to rule out possible effects of learning disability, ADHD, mood disorder, etc.
Includes: clinical interview, cognitive, achievement, memory and attention, fine motor skills, adaptive skills, parent/teacher/self-report forms on attention, behavior, and mood symptoms.
Adding an autism rule-out = $3600 total
ADHD evaluation - $2250
When academics are on-target and ADHD/behavior/mood is the main concern
Includes: clinical interview, cognitive, brief achievement, memory and attention, fine motor skills, parent/teacher/self-report forms on attention, executive functioning, behavior, and mood symptoms
Adult ADHD evaluation - $2250
For diagnostic clarification of symptoms
Includes: clinical interview, cognitive assessment, memory and attention, self-report forms on mood and behavior
ASD evaluation (ages 2+) - $2250
For initial diagnosis or later re-evaluation of symptoms and progress
Includes: clinical interview, cognitive/developmental assessment, ADOS-II, adaptive skills, parent/teacher/self-report forms on attention, behavior, and mood symptoms
IQ or Achievement only - $750
Commonly used to support a request for gifted services
Includes cognitive or achievement assessment and appropriate parent/teacher report form
Targeted evaluation – hourly estimate depending on time
For when there are additional diagnostic questions that have not been answered in previous evaluations or treatment
Tests determined based on clinical question
Payment
Payment is due up front at the time of service. You may pay by check, credit card, or Zelle transfer. While we cover the 3% service charge on credit card payments for therapy, we request payment through check or Zelle for assessments.
We will also provide you with invoices and superbills to submit to your insurance company for reimbursement of Out of Network benefits.
Good Faith Estimate
You have the right to receive a Good Faith Estimate (GFE) explaining how much your medical care will cost. At our office, this means:
Under the law, health care providers are required to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. This includes a total expected cost of any non-emergency items or services such as medical tests, prescription drugs, equipment, and hospital fees.
You should receive your GFE in writing at least 1 business day before your medical service, item, or appointment. You can also ask your health care provider for a GFE before you schedule an item or service.
If you receive a bill that is at least $400 more than your GFE, you can dispute the bill.
A copy of your GFE will be saved in your client portal.
